    <Abstract language="de" linked="yes"><Pgraph>Seit Verabschiedung durch die World Health Assembly im Jahre 2019 steht mit der 11. Revision der Internationalen statistischen Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD-11) die Abl&#246;sung der ICD-10 fest. Eine weitere Pflege der alten Revision erfolgt durch die Weltgesundheitsorganisation (WHO) nicht mehr. In Deutschland befasst sich das Kuratorium f&#252;r Fragen der Kodiersysteme im Gesundheitswesen (KKG) als Beratungsgremium des Bundesministeriums f&#252;r Gesundheit seit 2013 mit der ICD-11. Mit diesem Beitrag pl&#228;dieren Vertreter der Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften (AWMF) in der Arbeitsgruppe ICD-11 des KKG f&#252;r eine sorgf&#228;ltige und gut geplante, aber zeitnahe Umstellung. Die Umstellung von der ICD-10 auf die ICD-11 sichert die korrekte Abbildung des medizinischen Fortschritts in den kodierten Diagnosen und hebt die mit der Digitalisierung im Gesundheitswesen verbundenen Chancen f&#252;r Versorgung und Wissenschaft. Bei dieser Umstellung sollte konsequent das Ziel einer einmaligen Dokumentation medizinischer Sachverhalte bei mehrfacher Nutzung sowohl f&#252;r Versorgung und Forschung als auch f&#252;r Abrechnung und Gestaltung des Gesundheitswesens angestrebt werden. F&#252;r &#220;bergangszeiten mit paralleler Unterst&#252;tzung von ICD-10 und ICD-11 sowie f&#252;r die Bedienung weiterer <TextGroup><PlainText>Regel</PlainText></TextGroup>ungsb<TextGroup><PlainText>erei</PlainText></TextGroup>che &#8211; z.B. mit SNOMED CT &#8211; bedingt dies einen einmaligen Kodiervorgang in unterschiedliche Kodiersysteme. Hierzu bietet insbesondere der Einsatz generativer K&#252;nstlicher Intelligenz ein hohes Potential. Auch wenn die Absch&#228;tzung des Zeitbedarfs f&#252;r &#196;nderungen im deutschen Gesundheitssystem immer mit gro&#223;en Unsicherheiten behaftet ist, sollte diese Umstellung innerhalb der n&#228;chsten 5 Jahre m&#246;glich sein. Mit dann 13 Jahren ohne fachliche Pflege durch die WHO h&#228;tte die ICD-10 auch l&#228;ngst Anschluss an den medizinischen State of the Art verloren.</Pgraph></Abstract>
    <Abstract language="en" linked="yes"><Pgraph>Since its adoption by the World Health Assembly in 2019, the 11<Superscript>th</Superscript> Revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-11) has been set to replace ICD-10. The old revision is no longer maintained by World Health Organization (WHO). In Germany, a national advisory board (Kuratorium f&#252;r Fragen der Kodiersysteme im Gesundheitswesen (KKG)) of the Ministry of Health is concerned with ICD-11 since 2013. Members of the KKG Working Group ICD-11 from the Association of the Scientific Medical Societies in Germany (AWMF) describe in this paper the need for a careful, well planned and timely transition from ICD-10 to ICD-11. The transition to ICD-11 ensures a valid representation of medical progress with the coded <TextGroup><PlainText>diagnoses</PlainText></TextGroup> and seizes opportunities of digitization for health care and science. Combining a unique documentation of medical facts with their multiple use for daily health care  and research as well as for rei<TextGroup><PlainText>m</PlainText></TextGroup>bursement and health policy should be the ultimate goal. This goal demands a one-time coding covering different coding systems in order to bridge a transitional period with parallel support of ICD-10 and ICD-11 on the one hand and to support further policy areas, e.g. with SNOMED CT, on the other hand. In particular, the use of generative artificial intelligence offers a high potential to achieve this goal. Even though estimating the time required is always subject to a high degree of uncertainty given the nature of the German health care system, the transition from ICD-10 to ICD-11 can be carried out in the next 5 years. Then, 13 years without content maintenance through the WHO, the ICD-10 would have lost the connection to the medical State of the Art at the latest.</Pgraph></Abstract>

      <MainHeadline>1 Hintergrund</MainHeadline><Pgraph>Das Kuratorium f&#252;r Fragen der Kodiersysteme im Gesundheitswesen (KKG) befasst sich als Beratungsgremium des Bundesministeriums f&#252;r Gesundheit (BMG) mit der Weiterentwicklung von Kodiersystemen f&#252;r das Gesundheitswesen in Deutschland. Dazu unterh&#228;lt es Arbeitsgruppen zu ICD-10-GM und OPS. F&#252;r die Medizin benennt die Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften (AWMF) drei Mitglieder und jeweils zwei Stellvertretende f&#252;r das KKG. Der Vorsitz des KKG liegt derzeit bei der AWMF; Deutsche Krankenhausgesellschaft, GKV-Spitzenverband und Kassen&#228;rztliche Bundesvereinigung stellen jeweils stellvertretende Vorsitzende.</Pgraph><Pgraph>Das KKG ber&#228;t die Bundesregierung auch bez&#252;glich der nationalen Positionierung zu internationalen Entwicklungen. Vor diesem Hintergrund befasst sich das KKG seit 2013 intensiv mit der neuen, 11. Revision der Internationalen statistischen Klassifikation der Krankheiten und verwandter Gesundheitsprobleme (ICD-11) <TextLink reference="27"></TextLink>. Unter Beteiligung der AWMF wurde 2017 durch das KKG eine dedizierte Arbeitsgruppe (AG) ICD-11 etabliert. Am 28. und 29. Januar 2026 hat die AG ICD-11 in einem zweit<TextGroup><PlainText>&#228;gig</PlainText></TextGroup>en Workshop M&#246;glichkeiten der Einf&#252;hrung von SNOMED CT und ICD-11 in Deutschland diskutiert. Aus diesem Anlass m&#246;chten KKG-Mitglieder der AWMF den Stand und ihre Einsch&#228;tzungen zur Einf&#252;hrung der ICD-11 zusammenfassen.</Pgraph><Pgraph>Die ICD-11 wurde von der World Health Assembly (WHA) der Weltgesundheitsorganisation (WHO) 2019 verabschiedet <TextLink reference="9"></TextLink>. In Kraft getreten ist die ICD-11 am 1.1.2022; der Einsatz der ICD-11 f&#252;r statistische Auswertungen zu Mortalit&#228;t und Morbidit&#228;t wurde mit einer mindestens f&#252;nfj&#228;hrigen &#220;bergangszeit versehen. F&#252;r diese Zwecke hat die WHO eine Teilmenge der ICD-11 als ICD-11 for Mortality and Morbidity Statistics (ICD-11-MMS) ver&#246;ffentlicht. Die vom Bundesinstitut f&#252;r Arzneimittel und Medizinprodukte (BfArM) vorbereitete, aber noch nicht amtliche deutsche &#220;bersetzung der ICD-11-MMS (s. <TextLink reference="3"></TextLink>) ist seit Januar 2026 auch auf der WHO-Webseite inkl. einer Suchfunktion benutzbar, die das alphabetische Verzeichnis ersetzt <TextLink reference="30"></TextLink>.</Pgraph><Pgraph>Die einzige Alternative zur Einf&#252;hrung der ICD-11-MMS in Deutschland w&#228;re die unbefristete Fortf&#252;hrung der nationalen Modifikation der vorherigen Version (ICD-10-GM). Diese Alternative halten die AWMF-Mitglieder im KKG aus folgenden Gr&#252;nden nicht f&#252;r sinnvoll. Die ICD-11-MMS spiegelt den aktuellsten Stand des medizinischen Wissens wider. Dieser wurde in einem umfangreichen Entwicklungsprozess mit umfassender Beteiligung von medizinischen Expertinnen und Experten erarbeitet &#8211; auch aus Deutschland <TextLink reference="2"></TextLink>. Verz&#246;gerungen bei der Umstellung auf die ICD-11 f&#252;hren zudem zu einer Ausgrenzung Deutschlands von Initiativen zur Harmonisierung auf europ&#228;ischer und weltweiter Ebene. Die ICD-10 ist sp&#228;testens seit Einstellung ihrer Pflege durch die WHO fachlich abgeh&#228;ngt. Die Eintr&#228;ge bei der ICD-10 entsprechen weder dem aktuellen Wissen zu Erkrankungen, noch bilden sie die Indikationen des aktuellen Spektrums medizinischen Ma&#223;nahmen angemessen ab. Anpassungen der ICD-10-GM an nationale Gegebenheiten k&#246;nnen dies nicht ausgleichen. Auch in Deutschland wurde bereits auf ein dreij&#228;hriges Intervall der bisher j&#228;hrlichen &#220;<TextGroup><PlainText>b</PlainText></TextGroup>erar<TextGroup><PlainText>beitu</PlainText></TextGroup>ng umgestellt. </Pgraph><Pgraph>Die aktuellen Reformen des deutschen G<TextGroup><PlainText>esundhei</PlainText></TextGroup>tss<TextGroup><PlainText>ystem</PlainText></TextGroup>s (u.a. Krankenhausreform, elektronische Patientenakte, Prim&#228;rversorgungssystem) sind ein idealer Anlass f&#252;r die z&#252;gige Einf&#252;hrung der ICD-11-MMS. Bei Einf&#252;hrung zum 1.1.2027 w&#228;ren seit Start ihrer Entwicklung im Jahre 2007 auch bereits 20 Jahre vergangen <TextLink reference="17"></TextLink> &#8211; ein Zeitraum der den L&#228;ngen bei Entwicklung und Umsetzung der ICD-10 entsprechen w&#252;rde (s. Abbildung 1 <ImgLink imgNo="1" imgType="figure" />) <TextLink reference="12"></TextLink>, <TextLink reference="13"></TextLink>. F&#252;r Deutschland ist momentan jedoch noch kein konkreter Zeitpunkt f&#252;r eine Einf&#252;hrung der ICD-11-MMS festgelegt, weder f&#252;r eine Nutzung zur Kodierung von Todesursachen noch f&#252;r eine Abl&#246;sung der ICD-10-GM im Bereich der Sozialgesetzb&#252;cher.</Pgraph></TextBlock>
    <TextBlock name="1 Background" linked="yes" language="en">
      <MainHeadline>1 Background</MainHeadline><Pgraph>The National Board for Classification in Health Care at the Federal Ministry of Health (KKG) acts as an advisory body on all questions related to official classifications in the health care system. To this end, it maintains working groups on the German adaptation of the 10<Superscript>th</Superscript> Revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-10-GM) and the national procedure classification (OPS). For the medical sector, the Association of the Scientific Medical Societies in Germany (AWMF) appoints three members and two alternates each to the KKG. The AWMF currently holds the chairmanship of the KKG; the German Hospital Association, the National Association of Statutory Health Insurance Funds, and the National Association of Statutory Health Insurance Physicians each provide a vice-chair.</Pgraph><Pgraph>The KKG also advises the federal government on Germany&#8217;s national strategy regarding international developments. Against this backdrop, the KKG has been intensively engaged with the new, 11<Superscript>th</Superscript> revision of the ICD since 2013 <TextLink reference="27"></TextLink>. With the participation of the AWMF, a dedicated ICD-11 Working Group was established by the KKG in 2017. On January 28 and 29, 2026 the ICD-11 Working Group discussed options for the introduction of SNOMED CT and ICD-11 in Germany during a two-day workshop. On this occasion, KKG members of the AWMF would like to summarize the current status and their assessments regarding the introduction of ICD-11.</Pgraph><Pgraph>ICD-11 was adopted by the World Health Assembly (WHA) of the World Health Organization (WHO) in 2019 <TextLink reference="9"></TextLink>. ICD-11 entered into force on January 1, 2022; the use of ICD-11 for statistical analyses of mortality and morbidity was granted a transition period of at least five years. For these purposes, the WHO has published a subset of ICD-11 as ICD-11 for Mortality and Morbidity Statistics (ICD-11-MMS). The German translation of ICD-11-MMS, prepared by the Federal Institute for Drugs and Medical Devices (BfArM) but not yet official (see <TextLink reference="3"></TextLink>), has also been available on the WHO website since January 2026, including a search function that replaces the alphabetical index <TextLink reference="30"></TextLink>.</Pgraph><Pgraph>The only alternative to introducing the ICD-11-MMS in Germany would be the indefinite continuation of the national modification of the ICD-10. The AWMF members in the KKG do not consider this alternative to be sensible for the following reasons. The ICD-11-MMS reflects the latest state of medical knowledge, developed through an extensive process involving the broad participation of medical experts &#8211; including those from Germany <TextLink reference="2"></TextLink>. Delays in the transition to ICD-11 are also leading to Germany&#8217;s exclusion from harmonization initiatives at the European and global levels. The ICD-10 has been left behind in terms of content, at least since the WHO ceased its maintenance. The entries in the ICD-10 do not reflect current knowledge about diseases, nor do they adequately represent the indications for the current range of medical interventions. Adaptations of the ICD-10 to national conditions cannot compensate for this. As a consequence, the frequency of revisions has already been changed to a three-year cycle in Germany. </Pgraph><Pgraph>The current reforms of the German healthcare system (e.g. hospital reform, electronic health records, primary care system) provide an ideal opportunity for the rapid introduction of the ICD-11-MMS. However, if introduced on January 1, 2027, 20 years would have already passed since the start of its development in 2007 <TextLink reference="17"></TextLink> &#8211; a period corresponding to the time required for the development and implementation of ICD-10 (see Figure 1 <ImgLink imgNo="1" imgType="figure" />) <TextLink reference="12"></TextLink>, <TextLink reference="13"></TextLink>. For Germany, however, no specific date for the implementation of ICD-11 has yet been set, neither for use in coding causes of death nor for replacing the ICD-10-GM in the context of the Social Security Codes.</Pgraph></TextBlock>
    <TextBlock name="2 Kodierung von Diagnosen mit der ICD-11-MMS" linked="yes" language="de">
      <MainHeadline>2 Kodierung von Diagnosen mit der ICD-11-MMS</MainHeadline><Pgraph>Bei der ICD-11-MMS fallen sowohl &#196;hnlichkeiten als auch Unterschiede zur ICD-10 auf. Dies sollen drei Beispiele mit zunehmender Komplexit&#228;t belegen. In Tabelle 1 <ImgLink imgNo="1" imgType="table" /> ist die Kodierung der Diagnose &#8222;Appendizitis&#8220; dargestellt. Hier &#228;ndern sich nur Format und Aufbau des Kodes. Die ICD-11 erlaubt in der MMS bis zu 6 Stellen. F&#252;r die Nutzung der ICD-11 ist daher eine Auseinandersetzung mit ihrer Struktur und den formalen &#196;nderungen erforderlich, zumindest bei Wunsch nach Verst&#228;ndnis der Kodes. Dies erfordert Schulungs- und Fortbildungsangebote. Die M&#246;glichkeit zur Kodierung von Diagnosen mit fehlenden &#8211; und m&#246;glicherweise auch unzureichenden &#8211; Informationen bleibt auch bei der ICD-11-MMS bestehen.</Pgraph><Pgraph>Bei Kodierung der beidseitigen diabetischen Retinopathie fallen hingegen weitergehende Unterschiede auf (s. Tabelle 2 <ImgLink imgNo="2" imgType="table" />). So wird die M&#246;glichkeit zur Kombination von Kodes f&#252;r die Kodierung komplexer Diagnosen nicht mehr nur als Ausnahme (Kreuz-Stern-System der ICD-10), sondern als durchgehendes Prinzip angeboten, dem sogenannten Cluster Coding. Hierf&#252;r sieht die WHO ein Format zur Verkettung von Kodes vor: Stammkodes werden durch einen Schr&#228;gstrich (&#8222;&#47;&#8220;) verbunden, Zu<TextGroup><PlainText>s</PlainText></TextGroup>atzko<TextGroup><PlainText>d</PlainText></TextGroup>es an den entsprechenden Stammkode mit einem Et-Zeichen (&#8222;&#38;&#8220;) angeh&#228;ngt. Zudem sind Zusatzkodes durch ein f&#252;hrendes &#8222;X&#8220; zu erkennen. In Deutschland zus&#228;tzlich definierte Angaben wie die Seitenlokalisation wurden in die ICD-11-MMS integriert.</Pgraph><Pgraph>Das dritte Beispiel soll die nahezu grenzenlosen M&#246;glichkeiten der ICD-11-MMS zur Abbildung komplexester Sachverhalte &#8211; auch jenseits medizinischer Diagnosen &#8211; darstellen. Aus diesen M&#246;glichkeiten ergibt sich aber auch die Notwendigkeit von eindeutigen und nachvollziehbaren Verfahrensregelungen, wie sie die WHO in ihrem Coding Tool zur ICD-11-MMS festschreibt (s. <TextLink reference="30"></TextLink>), um eine einheitliche und den Zwecken angemessene Kodierung sicherzustellen. In diesem Beispiel handelt es sich um die &#8222;C6&#47;C7 Zerrei&#223;ungsfraktur nach Sturz im Pflegeheim&#8220; (s. Tabelle 3 <ImgLink imgNo="3" imgType="table" />). Bei der ICD-11-MMS k&#246;nnen auch die Umst&#228;nde dieser Verletzung abgebildet werden, und zwar so, dass nach diesen Umst&#228;nden bei statistischen Auswertungen recherchiert werden kann. Allerdings l&#228;sst sich weder in der ICD-10-GM noch in der ICD-11-MMS die Zerrei&#223;ung explizit abbilden, auch w&#228;re die Abgrenzung der Zerrei&#223;ung von einer Sch&#228;digung des Banda<TextGroup><PlainText>ppara</PlainText></TextGroup>ts und der Bandscheibe zu diskutieren. Auch bei der ICD-11-MMS besteht somit Bedarf an einem Schlie&#223;en von L&#252;cken. Die L&#228;nge der Kodesequenz mag auf den ersten Blick abschreckend wirken. Es ist aber davon auszugehen, dass bei Einf&#252;hrung der ICD-11 die Kodierung grunds&#228;tzlich mit Software-Unterst&#252;tzung erfolgt, so dass ein Erlernen der Kodes entf&#228;llt. Auch fordert die ICD-11-MMS die hier dargestellte Detailtiefe von Erkrankungsbezeichnungen nicht. Ebenso wie die ICD-10-GM bietet die ICD-11-MMS systematisch eine Kodierm&#246;glichkeit f&#252;r jeden gegebenen Informationsumfang durch die Bereitstellung spezifischer Klassen mit der Kennzeichnung &#8222;nicht n&#228;her bezeichnet&#8220; an.</Pgraph><Pgraph>Eine weitere, f&#252;r Deutschland wichtige Neuerung bei der ICD-11-MMS betrifft M&#246;glichkeiten zur Erg&#228;nzung von &#8222;Diagnosis Code Descriptors&#8220; wie der Diagnosesicherheit, Schweregrad, Typen von Entlassungsdiagnosen oder Zeitbez&#252;gen.</Pgraph></TextBlock>

      <MainHeadline>2 Coding of diagnoses with the ICD-11-MMS</MainHeadline><Pgraph>The ICD-11-MMS exhibits both similarities and differences compared to ICD-10. Three examples of increasing complexity are provided to illustrate this. Table 1 <ImgLink imgNo="1" imgType="table" /> shows the coding of the diagnosis &#8220;appendicitis.&#8221; Here, only the format and structure of the codes have changed. The ICD-11 allows for up to 6 digits. Therefore, using the ICD-11-MMS requires an understanding of its structure and the formal changes, at least if one wishes to comprehend the codes. This necessitates training and continuing education programs. The ability to code diagnoses with missing &#8211; and possibly insufficient &#8211; information remains available even with the ICD-11-MMS.</Pgraph><Pgraph>When coding bilateral diabetic retinopathy, however, more significant differences become apparent (see Table 2 <ImgLink imgNo="2" imgType="table" />). Thus, the option to combine codes for coding complex diagnoses is no longer offered merely as an exception (the &#8220;dagger and asterisk&#8221; system of ICD-10) but as a consistent principle, known as cluster coding. For this purpose, the WHO provides a format for concatenating codes: stem codes are connected by a slash (&#8220;&#47;&#8221;), and additional codes are appended to the corresponding stem code with an ampersand (&#8220;&#38;&#8221;). Moreover, additional codes are identified by a leading &#8220;X.&#8221; Information defined as particular data element in Germany, such as lateral localization, has been integrated into the ICD-11-MMS.</Pgraph><Pgraph>The third example is intended to illustrate the virtually limitless possibilities of the ICD-11-MMS for representing even the most complex situations &#8211; including those beyond medical diagnoses. However, these possibilities also necessitate clear and transparent procedural guidelines, such as those provided by the WHO in its ICD-11-MMS Coding Tool (see <TextLink reference="30"></TextLink>), to ensure consistent and appropriate coding. This example concerns a &#8220;C6&#47;C7 avulsion fracture following a fall in a nursing home&#8221; (see Table 3 <ImgLink imgNo="3" imgType="table" />). The ICD-11-MMS allows the circumstances of this injury to be recorded in such a way that statistical analyses can be conducted based on these circumstances. However, neither the ICD-10-GM nor the ICD-11-MMS explicitly captures the avulsion. It would also be worth discussing how to distinguish an avulsion from damage to the ligamentous apparatus and the intervertebral disc. Thus, there is a need to fill gaps even with the ICD-11-MMS. The length of the coding sequence may seem daunting at first glance. However, it can be assumed that upon the introduction of ICD-11, coding will be performed exclusively with software support, eliminating the need to learn the codes. Furthermore, ICD-11-MMS does not require this level of depth of coding for all use cases. As in ICD-10-GM, there are residual classes (&#8220;unspecified&#8221;).</Pgraph><Pgraph>Another new feature of the ICD-11 MMS that is important for Germany concerns options for adding diagnosis code descriptors, such as diagnosis certainty, severity, discharge diagnosis types, or timing information.</Pgraph></TextBlock>

      <MainHeadline>3 Schritte zu einem Einf&#252;hrungskonzept</MainHeadline><SubHeadline>Vorarbeiten</SubHeadline><Pgraph>Bereits vor der Abstimmung in der WHA 2019 hatte die AG ICD-11 des KKG Feldtests mit der Betaversion der ICD-11-MMS unter Beteiligung zahlreicher M<TextGroup><PlainText>itglie</PlainText></TextGroup>dsg<TextGroup><PlainText>esellschafte</PlainText></TextGroup>n der AWMF durchgef&#252;hrt <TextLink reference="27"></TextLink>. Dabei wurden 8 G&#252;tekriterien untersucht: Angemessenheit, Volls<TextGroup><PlainText>t&#228;ndigk</PlainText></TextGroup>eit, Konsistenz, Abweichungen zur ICD-10-GM, Prax<TextGroup><PlainText>i</PlainText></TextGroup>sta<TextGroup><PlainText>uglichkei</PlainText></TextGroup>t, Reliabilit&#228;t, Spezifit&#228;t und Anwendbarkeit. Kodierendes Personal sah in einer Befragung Vorteile der ICD-11-MMS gegen&#252;ber der ICD-10-GM bei Vollst&#228;ndigkeit und Praktikabilit&#228;t, Probleme hingegen in einem &#8211; ohne Nutzung von Zusatzkodes &#8211; zu geringen Detaillierungsgrad <TextLink reference="25"></TextLink>. Mit Unterst&#252;tzung durch das BMG wurden dann von Dezember 2019 bis Juni 2020 Workshops zu Umstiegsanalysen in vier Bereichen durchgef&#252;hrt: psychische St&#246;rungen, Schmerz, vertrags&#228;rztliche Nutzung sowie medizinische Register. Die Umstiegsanalysen erlaubten eine erste Absch&#228;tzung des Aufwands f&#252;r Vorlauf, Umsetzung und Nachlauf. Eine erweiterte Analyse wurde f&#252;r die Schweiz durchgef&#252;hrt <TextLink reference="11"></TextLink>. In Frankreich laufen Kodierungsstudien <TextLink reference="6"></TextLink>. Malaysia hat 2025 umgestellt <TextLink reference="20"></TextLink>. In Kanada laufen Vorarbeiten <TextLink reference="19"></TextLink>. Schweden hat einen Zeitplan zur Umstellung beschlossen (s. <TextLink reference="24"></TextLink>).</Pgraph><SubHeadline>Motivation</SubHeadline><Pgraph>Auf dem Workshop der AG ICD-11 am 26. und 27. Februar 2026 wurden die M&#246;glichkeiten zur Einf&#252;hrung der ICD-11-MMS in Deutschland insbesondere mit Blick auf den Einsatz von SNOMED CT als Terminologie bei der elektronischen Patientenakte nach &#167; 355 SGB V diskutiert. Hierbei scheiden aus Sicht der Autoren zwei Varianten aus. Eine doppelte Kodierung von Diagnosen durch &#196;rztinnen und &#196;rzte oder Kodierfachkr&#228;fte in ICD-11-MMS und SNOMED CT ist nicht praktikabel. Die hierf&#252;r erforderlichen Ressourcen stehen nicht zur Verf&#252;gung; Akzeptanz ist f&#252;r dieses Vorgehen nicht zu erwarten. Auch die Ableitung eines Kodiersystems aus dem jeweils anderen ist f&#252;r Diagnosen sowohl angesichts ihrer unterschiedlichen Konzepte <TextLink reference="23"></TextLink> als auch auf Grund methodischer Erw&#228;gungen <TextLink reference="16"></TextLink> nicht sinnvoll.</Pgraph><SubHeadline>Mehrfache Nutzung von Diagnoseangaben</SubHeadline><Pgraph>Schon beim Umstieg von ICD-9 auf ICD-10 wurde hingegen eine Anwendungszweck-abh&#228;ngige Kodierung auf Basis einer einheitlichen Repr&#228;sentation gesundheitsbezogener Sachverhalte vorgeschlagen (s. Abbildung 2 <ImgLink imgNo="2" imgType="figure" />) <TextLink reference="1"></TextLink>. Gesundheitsbezogene Sachverhalte werden dabei unter Einbezug der wesentlichen Anforderungen &#8211; von Ged&#228;chtnisst&#252;tze und Kommunikationshilfe bis hin zu Abrechnung, Qualit&#228;tssicherung (QS) und <TextGroup><PlainText>Versorgun</PlainText></TextGroup>gsfo<TextGroup><PlainText>rsch</PlainText></TextGroup>ung &#8211; einmalig in der medizinischen Dokumentation erfasst und dann f&#252;r verschiedene Anwendungszwecke in die jeweils geforderten Kodiersysteme unter Nutzung von anspruchsvollen Kodierkomponenten &#252;berf&#252;hrt. Auch das Prinzip der mehrfachen Verwendung von Daten ist nicht neu <TextLink reference="18"></TextLink>. Selbstverst&#228;ndlich sollten dabei m&#246;glichst viele Anwendungszwecke mit m&#246;glichst wenigen Kodiersystemen kombiniert werden. Aus Sicht der Gesundheitsforschung sind die Kodiersysteme jedoch nicht notwendigerweise auf SNOMED CT und ICD-11-MMS beschr&#228;nkt.</Pgraph><Pgraph>Zur Umsetzung des Prinzips document once &#8211; use many &#8211; code many ist wie beschrieben eine einheitliche Repr&#228;sentation gesundheitsbezogener Sachverhalte in der medizinischen Dokumentation unerl&#228;sslich. Zumindest m&#252;ssen alle zur Unterst&#252;tzung der Anwendungszwecke ben&#246;tigten Informationen verf&#252;gbar sein (z.B. der Befund abdomineller Schmerzen bei Angaben &#252;ber einen Notfall) und den erforderlichen Detaillierungsgrad erreichen (z.B. Angaben zur exakten Lokalisation abdomineller Schmerzen). Nur dann kann die medizinische Dokumentation Zwecke jenseits einer individuellen Ged&#228;chtnisst&#252;tze unterst&#252;tzen wie bei einer Diagnosestellung bei akutem Abdomen <TextLink reference="8"></TextLink>. </Pgraph><Pgraph>Die Anforderungen an dieses Prinzip vereinfachen sich jedoch bei Diagnosen (s. Abbildung 3 <ImgLink imgNo="3" imgType="figure" />). </Pgraph><Pgraph>Diagnosen werden voraussichtlich auch weiterhin nat&#252;rlichsprachig formuliert, so dass die freitextliche Beschreibung von Sachverhalten Ausgangspunkt f&#252;r den Kodiervorgang darstellt. Besteht f&#252;r die Kodierkomponenten Zugriff auf Inhalte elektronischer Akten, k&#246;nnen die dort verf&#252;gbaren Informationen sowohl zur Erg&#228;nzung, Konkretisierung und Validierung freitextlich formulierter Diagnosen als auch zur automatisierten Ableitung (&#8222;derive once&#8220;) von Diagnosen genutzt werden. Die gleichzeitige Kodierung der Diagnoseangaben in mehrere Kodiersysteme kann dann durch Methoden der k&#252;nstlichen Intelligenz (KI), insbesondere unter Einsatz gro&#223;er Sprachmodelle (Large Language Models, LLMs) unterst&#252;tzt werden. Allerdings setzt die ICD-11-MMS selbst Bedingungen, die neben den Anforderungen aus den Anwendungszwecken bei der Formulierung von Diagnosen zu beachten sind <TextLink reference="28"></TextLink>. Diese Bedingungen sind von den IT-Systemen in Krankenh&#228;usern und Arztpraxen in Form einer transparenten Unterst&#252;tzung des Dokumentationsprozesses einzubinden. Beim Einsatz generativer KI sind Vorkehrungen gegen Verzerrungen der Kodierergebnisse insbesondere in Bezug auf Diversit&#228;t, Gleichheit und Inklusion (englisch Diversity, Equity, and Inclusion) zu treffen, wie es auch gilt, falsche Ergebnisse durch Fabulieren der KI zu unterbinden.</Pgraph><SubHeadline>Voraussetzungen</SubHeadline><Pgraph>Mit Blick auf das derzeitige Angebot der WHO zur Nutzung der ICD-11-MMS sind f&#252;r eine Umsetzung dieses Prinzips zwei Bedingungen festzuhalten. Die Hoheit &#252;ber freitextliche Angaben einer Diagnose, &#252;ber die Inhalte einer ggf. zugreifbaren elektronischen Akte sowie &#252;ber das von Kodierkomponenten vorgeschlagene Ergebnis einer Kodierung in ICD-11-MMS und SNOMED CT muss bei der f&#252;r die Dokumentation verantwortlichen Einrichtung liegen. Zentrale und nicht komplett nachvollziehbare Systeme k&#246;nnen daher nur Referenzcharakter haben; das konkrete Angebot f&#252;r digitale L&#246;sungen zur Unterst&#252;tzung des Prinzips use many &#8211; code many sollte in einem Wettbewerb interoperabler Komponenten liegen, bei denen die Einhaltung wichtiger Qualit&#228;tskriterien durch eine Zertifizierung oder vergleichbare Verfahren sichergestellt wird. Dabei sollte auch die Kompatibilit&#228;t der D<TextGroup><PlainText>a</PlainText></TextGroup>tenf<TextGroup><PlainText>orm</PlainText></TextGroup>ate sichergestellt werden.</Pgraph><Pgraph>Eine wettbewerbliche Infrastruktur von Kodierkomponenten setzt die uneingeschr&#228;nkte Bereitstellung aller Materialien durch die zust&#228;ndigen Stellen voraus, auch als sogenanntes &#8222;Computable Knowledge&#8220; <TextLink reference="5"></TextLink>. Dies betrifft sowohl die WHO f&#252;r eine Referenzfassung der ICD-11-MMS als auch das BfArM f&#252;r eine eventuelle nationale Anpassung. Nur bei Bereitstellung aller Materialien k&#246;nnen Dritte L&#246;sungen entwickeln, die unabh&#228;ngig von der derzeit durch die WHO angebotenen Programmierschnittstelle (ICD-API) funktionieren. Vorbild kann hierbei das Angebot des BfArM f&#252;r ICD-10 und OPS sein. Dies umfasst sowohl eine systematische Darstellung der Klassifikationen als PDF-Datei, wie auch eine niedrigschwellige Listung der Inhalte zur automatisierten Weiterverarbeitung (sogenannte Metadaten) und eine umfassende Repr&#228;sentation aller Inhalte mit der Classification Markup Language (ClaML) als Ausgangspunkt f&#252;r wettbewerbliche Kodierkomponenten. Dieser Umfang an Materialien w&#228;re allerdings f&#252;r die ICD-11-MMS zu erweitern, ohne dass an dieser Stelle auf alle Aspekte eingegangen werden kann (z.B. in Bezug auf die &#8222;WHO-FIC Foundation&#8220;, s. <TextLink reference="29"></TextLink>). Aus Sicht der Autoren kann der Weg zum richtigen K<TextGroup><PlainText>odi</PlainText></TextGroup>erer<TextGroup><PlainText>gebn</PlainText></TextGroup>is &#8211; z.B. die Auswahlliste zu einem gegebenen Diagnosentext &#8211; nicht Gegenstand von verpflichtenden Festlegungen sein. Effizienz, Methoden und Vorgehen zur Ermittlung des korrekten Ergebnisses einer Diagnosekodierung w&#228;ren sowohl bei ICD-11-MMS als auch bei SNOMED CT den eingesetzten Kodierkomponenten &#252;berlassen, so wie jetzt bei der ICD-10-GM.</Pgraph><Pgraph>Mit Blick auf die Resilienz unsere Gesundheitssystems sollte eine Kodierung von Diagnosen auch bei Ausfall zentraler Referenzinstitutionen m&#246;glich sein. Sowohl in der t&#228;glichen Routine als auch im Krisenfalle ist die Klassifikation von Diagnosen f&#252;r Abl&#228;ufe und Abstimmungsprozesse der individuellen Versorgung wie auch zur Triage und Ressourcenallokation auf Systemebene zu elementar, um von einer durchgehenden Verf&#252;gbarkeit der Ressourcen von WHO, BfArM oder SNOMED International abh&#228;ngig zu sein. F&#252;r die komplexe &#220;<TextGroup><PlainText>berga</PlainText></TextGroup>ngsph<TextGroup><PlainText>a</PlainText></TextGroup>se von der ICD-10 zur ICD-11-MMS sind allerdings in jedem Falle Vorkehrungen f&#252;r m&#246;gliche Ausf&#228;lle und St&#246;rungen zu treffen.</Pgraph><SubHeadline>Nationale Anpassungen der ICD-11-MMS</SubHeadline><Pgraph>Die Vermeidung nationaler und mit der Fassung der WHO inkompatibler Ableitungen zur ICD war ein Ziel bei der Entwicklung der ICD-11. Ein Beitrag hierzu war die Definition von Linearizations, die Teile der umfassenden Foundation f&#252;r bestimmte Anwendungszwecke zusammenfassen. Beispiele hierf&#252;r sind die ICD-11-MMS sowie eine m&#246;gliche ICD-11 f&#252;r Primary Care (ICD-11-PHC). Die Nutzung einer gemeinsamen Grundlage zur Auspr&#228;gung anwendungsbezogener Ausschnitte stellt dabei die Kompatibilit&#228;t kodierter Daten sicher, auch wenn bei einer Zusammenf&#252;hrung immer die jeweils abstraktere Ebene herangezogen werden m&#252;sste. Es bleibt aber die Herausforderung, nationale, administrative Gegebenheiten jenseits der Festlegung medizinischer Sachverhalte abzubilden. Der hierzu mit der ICD-10-GM beschrittene Weg einer insbesondere auf Abrechnungszwecke optimierten Fassung muss daher f&#252;r die ICD-11 hinterfragt werden. Entsprechende nationale Anpassungen w&#252;rden die Anforderungen einer internationalen Harmonisierung verkennen, insbesondere auf europ&#228;ischer Ebene mit dem European Health Data Space (EHDS) <TextLink reference="22"></TextLink>. Mit der 11. Revision verbundene Chancen zur Nutzung der ICD in multinationalen Forschungsprojekte blieben durch Mappingprobleme zwischen m&#246;glicherweise inkompatiblen, nationalen Fassungen ungenutzt. Zudem ist bei nationalen Erweiterungen stets auf die jeweilige Abw&#228;rtskompatibilit&#228;t zu achten. Die Einf&#252;hrung der ICD-11-MMS sollte daher von einem Umdenken bez&#252;glich nationaler Anpassungen verbunden sein. W&#228;hrend die ICD-10-GM &#8211; auch auf Grund von Beschr&#228;nkungen in Konzeption und Struktur &#8211; von der Selbstverwaltung als Modelliermasse zur m&#246;glichst einfachen Umsetzung insbesondere abrechnungstechnischer Aspekte betrachtet wurde, sollten Tiefe und Umfang der ICD-11 eine so pr&#228;zise Abbildung von Diagnosen erm&#246;glichen, dass sich abrechnungstechnische Aspekte an den vorhandenen Eintr&#228;gen orientieren. Mit dem Sachverhalt einer medizinischen Diagnose nicht verbundene Angaben sind dann au&#223;erhalb einer ICD-11-MMS zu regeln. Der Umfang von Routinedaten w&#228;re f&#252;r Aspekte wie externe Qualit&#228;tssicherung, G<TextGroup><PlainText>esundhei</PlainText></TextGroup>tsbe<TextGroup><PlainText>richterstatt</PlainText></TextGroup>ung und Versorgungsforschung entsprechend zu erweitern.</Pgraph></TextBlock>
    <TextBlock name="3 Steps toward an implementation plan" linked="yes" language="en">
      <MainHeadline>3 Steps toward an implementation plan</MainHeadline><SubHeadline>Preliminary work</SubHeadline><Pgraph>Even before the vote at the WHA 2019, the ICD-11 Working Group had conducted field tests with the beta version of the ICD-11-MMS with the participation of numerous member societies of the AWMF <TextLink reference="27"></TextLink>. Eight quality criteria were examined: appropriateness, completeness, consistency, deviations from the ICD-10-GM, practicality, reliability, specificity, and applicability. In a survey, coding staff identified advantages of the ICD-11-MMS over the ICD-10-GM in terms of completeness and practicality, but also noted problems with a level of detail that was too low &#8211; without the use of additional codes <TextLink reference="25"></TextLink>. With support from the Federal Ministry of Health, workshops on transition analyses were conducted from December 2019 to June 2020 in four areas: mental disorders, pain, use by panel physicians, and medical registries. The transition analyses allowed for an initial assessment of the effort required for preparation, implementation, and maintenance. An expanded analysis was conducted for Switzerland <TextLink reference="11"></TextLink>. Coding studies are underway in France <TextLink reference="6"></TextLink>. Malaysia completed its transition in 2025 <TextLink reference="20"></TextLink>. Preparatory work is underway in Canada <TextLink reference="19"></TextLink>. Sweden has adopted a timeline for the transition (see <TextLink reference="24"></TextLink>).</Pgraph><SubHeadline>Motivation</SubHeadline><Pgraph>At the ICD-11 Working Group workshop on February 26 and 27, 2026, the possibilities for introducing the ICD-11-MMS in Germany were discussed, particularly with regard to the use of SNOMED CT as the terminology for the electronic health record pursuant to Section 355 of the German Social Code, Book V (SGB V). From the authors&#8217; perspective, two options are ruled out. Dual coding of diagnoses by physicians or coding specialists in both ICD-11-MMS and SNOMED CT is not feasible. The resources required for this are not available; acceptance of this approach is not expected. Deriving one coding system from the other is also not feasible for diagnoses, both in light of their differing concepts <TextLink reference="23"></TextLink> and due to methodological considerations <TextLink reference="16"></TextLink>.</Pgraph><SubHeadline>Multiple use of diagnostic data </SubHeadline><Pgraph>However, even during the transition from ICD-9 to ICD-10, application-purpose-specific coding based on a uniform representation of health-related facts was proposed (see Figure 2 <ImgLink imgNo="2" imgType="figure" />) <TextLink reference="1"></TextLink>. Health-related facts are recorded once in medical documentation, taking into account the essential requirements &#8211; from memory aids and communication tools to billing, quality assurance (QA), and health services research and then transferred to the respective coding systems required for the various purposes using sophisticated coding components. The principle of multiple data use is also not new <TextLink reference="18"></TextLink>. Naturally, as many application purposes as possible should be combined with as few coding systems as possible. From the perspective of health research, however, the coding systems are not necessarily limited to SNOMED CT and ICD-11-MMS.</Pgraph><Pgraph>To implement the &#8220;document once &#8211; use many &#8211; code many&#8221; principle, a uniform representation of health-r<TextGroup><PlainText>elat</PlainText></TextGroup>ed facts in medical documentation is essential, as described. At a minimum, all information required to support the intended uses must be available (e.g., findings of abdominal pain in reports of an emergency) and must reach the necessary level of detail (e.g., information on the exact location of abdominal pain). Only then can medical documentation support purposes beyond serving as an individual memory aid, such as in making a diagnosis of acute abdomen <TextLink reference="8"></TextLink>. However, the requirements for this principle are simplified in the case of diagnoses (see Figure 3 <ImgLink imgNo="3" imgType="figure" />).</Pgraph><Pgraph>Diagnoses are expected to continue to be formulated in natural language, so that the free-text description of facts serves as the starting point for the coding process. If the coding components have access to the contents of electronic records, the information available there can be used to supplement, specify, and validate free-text diagnoses as well as to automatically derive (&#8220;derive once&#8221;) diagnoses. The simultaneous coding of diagnostic data into multiple coding systems can then be effectively supported by artificial intelligence (AI) methods, particularly through the use of large language models (LLMs). Moreover, the ICD-11-MMS itself sets conditions that must be observed in addition to the requirements arising from the intended uses when formulating diagnoses <TextLink reference="28"></TextLink>. These conditions must be integrated into the information systems in hospitals and medical practices in the form of transparent support for the documentation process. When using generative AI, precautions must be taken to prevent bias in the model&#8217;s outputs, particularly with regard to diversity, equity, and inclusion (DEI), just as measures must be taken to prevent incorrect results arising from the model&#8217;s confabulations.</Pgraph><SubHeadline>Prerequisites</SubHeadline><Pgraph>However, in light of the WHO&#8217;s current guidelines for using the ICD-11-MMS, two conditions must be noted for the implementation of this principle. Control over free-text diagnoses entries, over the contents of any accessible electronic records, and over the coding results proposed by coding components in ICD-11-MMS and SNOMED CT must lie with the institution responsible for the documentation. Centralized and not fully traceable systems can therefore serve only as references; the concrete offering of digital solutions to support the &#8220;use many &#8211; code many&#8221; principle should be based on a competitive market of interoperable components, where compliance with key quality criteria is ensured through certification or comparable procedures. The compatibility of data formats must be ensured in this context.</Pgraph><Pgraph>However, a competitive infrastructure of coding components requires the unrestricted provision of all materials by the responsible authorities, including so-called &#8220;Computable Knowledge&#8221; <TextLink reference="5"></TextLink>. This applies both to the WHO for a reference version of the ICD-11-MMS and to the BfArM for any potential national adaptation. Only if all materials are made available can third parties develop solutions that function independently of the programming interface (ICD-API) currently offered by the WHO. The BfArM&#8217;s offering for ICD-10-GM and OPS can serve as a model here. The current support includes both a systematic presentation of the classifications as a PDF file, as well as a low-threshold listing of the content for automated further processing (so-called metadata) and a comprehensive representation of all content using the Classification Markup Language (ClaML) as a starting point for competitive coding components. However, this scope of materials would need to be expanded for the ICD-11-MMS, without it being possible to address all aspects here (e.g. regarding the &#8220;WHO-FIC Foundation,&#8221; see <TextLink reference="29"></TextLink>). Additionally, from the authors&#8217; perspective, the path to the correct coding result &#8211; e.g. the selection list for a given diagnosis text &#8211; cannot be the subject of mandatory specifications. Efficiency, methods, and procedures for determining the correct result of a diagnosis coding would be left to the coding components used in both ICD-11-MMS and SNOMED CT, as is currently the case with ICD-10-GM.</Pgraph><Pgraph>With a view to the resilience of our healthcare system, it must be possible to code diagnoses even in the event of a failure of central reference institutions. Both in daily routine and in crisis situations, the classification of diagnoses is too fundamental for workflows and coordination processes in individual care, as well as for triage and resource allocation at the system level, to be made dependent on the continuous availability of resources from the WHO, BfArM, or SNOMED International. Therefore, during the complex transition phase from ICD-10 to ICD-1-MMS, precautions must be taken in any case to address potential system failures.</Pgraph><SubHeadline>National adaptations of the ICD-11-MMS</SubHeadline><Pgraph>A key objective in the development of ICD-11 was to avoid national adaptations of the ICD that are incompatible with the WHO version. One step toward this goal was the definition of &#8220;linearizations&#8221;, which consolidate parts of the comprehensive foundation for specific applications. Examples include the ICD-11-MMS and a potential ICD-11 for Primary Care (ICD-11-PHC). The use of a common basis for defining application-specific subsets ensures the compatibility of coded data, even though the more abstract level would always have to be used when merging the data. However, the challenge remains of reflecting national administrative circumstances beyond the definition of medical conditions. The approach taken with the ICD-10-GM, which involves a national version optimized specifically for billing purposes, must therefore be reevaluated for ICD-11. Such national adaptations would fail to recognize the requirements of international harmonization, particularly at the European level with the European Health Data Space (EHDS) <TextLink reference="22"></TextLink>. Opportunities associated with the 11<Superscript>th</Superscript> revision for using the ICD in multinational research projects remained untapped due to mapping problems between potentially incompatible national versions. In addition, when implementing national extensions, it is always important to ensure backward compatibility. The introduction of the ICD-11-MMS should therefore be accompanied by a rethinking of national adaptations. While the ICD-10-GM &#8211; partly due to limitations in its design and structure &#8211; was viewed by the self-governing bodies as a malleable tool for the simplest possible implementation, particularly of billing-related aspects, the depth and scope of the ICD-11 should enable such a precise representation of diagnoses that billing-related aspects are based on the existing entries. Information not related to the facts of a medical diagnosis should then be regulated outside of an ICD-11-MMS. The scope of routine data should then be expanded accordingly to cover areas such as external quality assurance, national statistics, and health services research.</Pgraph></TextBlock>
    <TextBlock name="4 Diskussion und Ausblick" linked="yes" language="de">
      <MainHeadline>4 Diskussion und Ausblick</MainHeadline><Pgraph>Der von den medizinisch-wissenschaftlichen Fachgesellschaften vertretene Bereich gesundheitsbezogener Forschung ist auf eine internationale Klassifikation f&#252;r Erkrankungen angewiesen, die den Stand der Wissenschaft abbildet. Ein Festhalten an einer veralteten und nicht mehr gepflegten ICD-10 ist daher f&#252;r von der AWMF entsandte Mitglieder der AG ICD-11 des KKG keine Option. Dies ist f&#252;r die fast vollst&#228;ndig auf ICD-kodierten Daten beruhende Qualit&#228;tssicherung und Versorgungsforschung zu Krebserkrankungen offensichtlich <TextLink reference="31"></TextLink>. Auch die Internationale Klassifikation der Krankheiten f&#252;r die Onkologie (ICD-O) ist dabei, den &#220;bergang zur ICD-11 nachzuvollziehen. Die Sekund&#228;rnutzung von Gesundheitsdaten im EHDS kann bei &#220;bergang auf die ICD-11 mit zuverl&#228;ssigeren und detailreicheren Daten rechnen, die durch die neuen M&#246;glichkeiten zur Kombination von Stamm- und Zusatzkodes sinnvollere Zusammenh&#228;nge abbilden und neue M&#246;glichkeiten des Retrievals bieten. Auch klinische Studien profitieren, z.B. &#252;ber eine zeitgem&#228;&#223;e Repr&#228;sentation von Diagnosen, die als Ein- und Ausschlusskriterien zur Rekrutierung hohe Bedeutung haben <TextLink reference="10"></TextLink>.</Pgraph><Pgraph>Eine Abl&#246;sung von ICD-10 und ICD-10-GM ist aber auch zur Erf&#252;llung der Aufgaben der Sozialgesetzb&#252;cher zwingend. So l&#228;sst sich die Aufgabe, &#8222;Gesundheit der Versicherten zu erhalten, wiederherzustellen oder ihren Gesundheitszustand zu bessern&#8220; im SGB V von den Einrichtungen der Selbstverwaltung kaum auf Grundlage von Angaben zu Diagnosen erf&#252;llen, die medizinisch nicht dem Stand des Wissens entsprechen. Eine Petition an den deutschen Bundestag unterstreicht die Bedeutung eines Umstiegs aus Sicht von Patientinnen und Patienten <TextLink reference="21"></TextLink>. Unmittelbare Vorteile durch Umstellung auf die ICD-11-MMS erg&#228;ben sich &#252;berall dort, wo Diagnosekodes einen wichtigen Beitrag zur Kommunikation und Versorgungsgestaltung leisten. Dies betrifft u.a. die Verordnung von Krankenhausbehandlung mit Kodierung von Diagnosen gem&#228;&#223; &#167; 295 SGB V, die Auswahl eines Krankenhauses unter Zuhilfenahme der Qualit&#228;tsberichte nach &#167; 136 SGB V mit Leistungszahlen f&#252;r Diagnosekodes oder die Nutzung von Diagnosekodes in Epikrisen zur eindeutigen und unmissverst&#228;ndlichen Referenzierung freitextlicher Erkrankungsbeschreibungen. Bei einer automatisierten Pr&#252;fung der Arzneimitteltherapiesicherheit (AMTS) durch Abgleich ICD-kodierten Diagnose- und Allergieinformationen mit Indikationen, Kontraindikationen und Angaben zu Wechselwirkungen aus Fachinformationen im Rahmen elektronische Verordnungssysteme (englisch Computerized Physician Order Entry) nach &#167; 342 SGB V gewinnt die Kodierung von Diagnosen lebensrettende Bedeutung. </Pgraph><Pgraph>Die f&#252;r eine &#220;bergangsphase denkbare Bedienung der ICD-10-GM als ein Kodiersystem neben ICD-11-MMS und SNOMED CT &#228;ndert an dessen fundamentaler Veralterung nicht. Von der Diagnosekodierung abh&#228;ngige Systeme wie die Qualit&#228;tssicherung nach &#167; 137 SGB V, die Definition der aG-DRG oder der Risikostrukturausgleich sollten daher nach einer m&#246;glichst kurzen Frist in die ICD-11-MMS &#252;berf&#252;hrt werden. Anderenfalls verfehlen diese Systeme die mit ihnen verbundenen Ziele. Allerh&#246;chsten f&#252;r einen kurzen Zeitraum k&#246;nnten die Kodes der ICD-10-GM aus den ICD-11-Daten automatisiert ermittelt werden (s. Abbildung 4 <ImgLink imgNo="4" imgType="figure" />). F&#252;r ein Mapping zwischen ICD-10-GM und ICD-11-MMS im Kontext der Systeme sind verschiedene Optionen denkbar <TextLink reference="26"></TextLink>.</Pgraph><Pgraph>Die ICD-11-MMS verbessert die Abbildung von Diagnosen &#252;ber eine Klassifikation. Als Bestandteil eines zunehmend digital unterst&#252;tzten Gesundheitssystems sind mit ihrem Einsatz keine neuen b&#252;rokratischen Anforderungen &#252;ber die Einf&#252;hrungsphase hinaus verbunden <TextLink reference="14"></TextLink>. Die Einf&#252;hrung der ICD-11-MMS sollte daher ohne die Festlegung zus&#228;tzlicher Regularien erfolgen. Bestehende Anwendungsszenarien von ICD-10 und ICD-10-GM k&#246;nnen uneingeschr&#228;nkt mit der ICD-11-MMS fortgesetzt werden, ohne dass zus&#228;tzliche Vorgaben erforderlich w&#228;ren. Die Einf&#252;hrung der ICD-11-MMS sollte daher aus Sicht der Autoren nicht mit zus&#228;tzlichen Umstellungen verbunden sein, weder f&#252;r die in den Kodierprozess unmittelbar einbezogene Personen noch f&#252;r Nutzer der kodierten Diagnosen. Die Qualit&#228;t der Kodierung kann bei der ICD-11 mit den gleichen Verfahren sichergestellt werden, die bereits bei der ICD-10 zum Einsatz kommen. So komplex die ICD-11 auf den ersten Blick auch scheint, ihre Einf&#252;hrung sollte schlank und aufwandsarm f&#252;r die Gesundheitsberufe gestaltet werden <TextLink reference="11"></TextLink>. Das BfArM selbst hat unabh&#228;ngig von einzelnen Kodiersystemen ein Positionspapier zu einer Semantikstrategie f&#252;r das deutsche Gesundheitswesen vorgelegt <TextLink reference="4"></TextLink>.</Pgraph><Pgraph>Bei der Diskussion um ICD-10, ICD-11 und SNOMED CT sollte die zentrale Rolle der Diagnose (und der medizinischen Dokumentation) f&#252;r den medizinischen Behandlungsprozess nicht vergessen werden. Als Arbeitsdiagnose <TextLink reference="7"></TextLink>, <TextLink reference="15"></TextLink> ist sie das Ergebnis eines &#228;rztlichen Erkenntnisprozesses und leitend f&#252;r die Festlegung eines Behandlungsplanes. Schon jetzt ist hierbei eine Pr&#228;gung durch die ICD-10 wahrscheinlich. Umso wichtiger ist es, sich inhaltlich mit der ICD-11 auseinanderzusetzen und ihre Einf&#252;hrung praxisgerecht auszugestalten. U.a. z&#228;hlen hierzu die Identifizierung von fachlichen Erweiterungen der ICD-10-GM, die in der ICD-11-MMS noch fehlen, oder Inhalten der ICD-11-MMS, die f&#252;r Zwecke des SGB V nicht erforderlich sind. Dem Ziel einer sowohl praxistauglichen als auch die Herausforderungen eines digitalen Gesundheitswesens adressierenden ICD f&#252;hlen sich die Autoren als Mitglieder der AG ICD-11 des KKG verpflichtet.</Pgraph></TextBlock>
    <TextBlock name="4 Discussion and outlook" linked="yes" language="en">
      <MainHeadline>4 Discussion and outlook</MainHeadline><Pgraph>The field of health-related research represented by medical-scientific professional societies relies on an international classification of diseases that reflects the state of the art in science. Clinging to an outdated and no longer maintained ICD-10 is therefore not an option for members of the KKG&#8217;s ICD-11 Working Group delegated by the AWMF. This is evident in the fields of quality assurance and health services research on cancer, which rely almost entirely on ICD-coded data <TextLink reference="31"></TextLink>. The International Classification of Diseases for Oncology (<TextGroup><PlainText>ICD-O</PlainText></TextGroup>) is in the process of making the transition to ICD-11. The secondary use of health data in the EHDS can expect more reliable and detailed data following the transition to ICD-11, as the new options for combining stem and additional codes will enable the identification of more meaningful correlations and offer new retrieval possibilities. Clinical trials also benefit, for example, from an up-to-date representation of diagnoses that are of great importance as eligibility criteria <TextLink reference="10"></TextLink>.</Pgraph><Pgraph>However, replacing ICD-10 and ICD-10-GM is also imperative for fulfilling the mandates of the Social Security Codes. For instance, the mandate to &#8220;maintain, restore, or improve the health of insured persons&#8221; in SGB V can hardly be fulfilled by self-governing institutions based on diagnoses data that does not reflect the current state of medical knowledge. A petition to the German Bundestag highlights the need for a transition from the perspective of patients <TextLink reference="21"></TextLink>. An immediate benefit of migrating to the ICD-11-MMS would arise wherever diagnoses codes play a key role in the communication and the organization of care. This applies, amongst other things, to the prescription of hospital treatment with the coding of diagnoses in accordance with Section 295 of SGB V, the selection of a hospital using the quality reports under Section 136 of SGB V that assign performance figures to diagnoses codes, or the use of diagnoses codes in discharge summaries and other epicrises to provide clear and unambiguous references for free-text descriptions of medical conditions. In the context of medication safety, the coding of diagnoses takes on life-saving importance through cross-referencing ICD-coded diagnoses and allergy information against indications, contraindications, and details of interactions taken from summaries of product characteristics within computerized physician order entry systems in accordance with Section 342 of SGB V.</Pgraph><Pgraph>The use of ICD-10-GM as a coding system alongside ICD-11-MMS and SNOMED CT, which is conceivable for a transitional period, does not alter its fundamental weaknesses. Systems dependent on diagnoses coding, such as quality assurance under Section 137 of SGB V, the definition of the German Diagnoses Related Groups (aG-DRG), or the morbidity-based risk adjustment, must therefore be migrated to ICD-11-MMS as soon as possible. Otherwise, these systems will fail to achieve their intended objectives. However, for a transitional phase, the ICD-10-GM codes could also be delivered by an AI supported coding process (see Figure 4 <ImgLink imgNo="4" imgType="figure" />). There are various options for a mapping between ICD-10-GM and ICD-11-MMS in the context of the mentioned systems <TextLink reference="26"></TextLink>.</Pgraph><Pgraph>The ICD-11-MMS improves the representation of diagnoses through a classification system. As part of an increasingly digitally supported healthcare system, its implementation does not entail new bureaucratic requirements beyond the transition phase <TextLink reference="14"></TextLink>. Therefore, the introduction of the ICD-11-MMS should not be linked to additional regulations. Existing application scenarios for ICD-10 and ICD-10-GM can be continued using the ICD-11-MMS, without the need for additional regulations. From the authors&#8217; perspective, the introduction of the ICD-11-MMS should therefore not entail any additional changes, neither for those directly involved in the coding process nor for users of the coded diagnoses. The quality of coding in ICD-11 can be ensured using comparable procedures already in use for ICD-10. As complex as ICD-11 may seem at first glance, its introduction should be designed to be streamlined and low-effort for healthcare professionals <TextLink reference="11"></TextLink>. The BfArM itself has presented a position paper on a semantic strategy for the German healthcare system, independent of a specific coding system <TextLink reference="4"></TextLink>.</Pgraph><Pgraph>In the discussion regarding ICD-10, ICD-11, and SNOMED CT, the central role of the diagnosis (and medical documentation) in the medical treatment process should not be forgotten. As an intermediate diagnosis <TextLink reference="7"></TextLink>, <TextLink reference="15"></TextLink>, it is the result of a medical diagnostic process and guides the establishment of a treatment plan. It is already likely that ICD-10 had an influence here. This makes it all the more important to engage with the content of ICD-11 and to design its implementation in a way that is practical and relevant to clinical practice. This includes, among other things, the identification of content of the ICD-10-GM that is still missing from the ICD-11-MMS, or content of the ICD-11-MMS that is not required for the purposes of SGB V. As members of the KKG&#8217;s ICD-11 Working Group, the authors are committed to the goal of developing an ICD that is both designed for use in everyday work and responsive to the challenges of a digital healthcare system.</Pgraph></TextBlock>
